Clinical Support 

AmerisourceBergen Clinical ServicesWhen a patient requires clinical support, his or her care should be handled by someone trusted and knowledgeable. By connecting the hospice with local pharmacy providers, the AmerisourceBergen Pharmacy to Hospice (PHA) program ensures the patient receives care from providers who understand the situation and can make the best recommendation. Whether the hospice is adjusting patient therapy or satisfying the CMS conditions of participation, the PHA program supports the provision of these services.

The level of clinical support is customized to the needs of the hospice agency as determined between the primary pharmacy and the hospice to support their goals and objectives.

The clinical role of the PHA pharmacist can often include:

  • Participation in Patient & Therapeutics Committees to support the need for medication management
  • Selective review of the patient’s medication history to conduct drug medication therapy reviews as required by CMS and as directed by the hospice agency
  • Participation in individual patient reviews when drugs outside of the formulary are prescribed or when drugs over a threshold amount are prescribed

Clinical services will vary and are negotiated between the individual hospice agency and PHA pharmacy.
