AmerisourceBergen recognizes that these are challenging times for our pharmacy customers and for local hospice agencies across the country. Just as independent pharmacies are constrained by national chains and reimbursement, hospice agencies are threatened by external business pressures that put them at risk. Relying on the knowledge, reach and partnership of AmerisourceBergen, we developed the Pharmacy to Hospice Administration program to solidify and enhance the relationships between these two key constituents to allow for increased profitability and better patient outcomes. Partnership opportunities as offered by this solution create a working relationship that can sustain the success of each partner.

Through this Pharmacy to Hospice Administration program, AmerisourceBergen provides administrative tools (claims processing, eligibility management, formulary management, network access, cash management, reporting and more) to local pharmacies so that they can devote more time and resources to dispensing medications and expanding their offerings to local hospice agencies. Creating a direct partnership between the pharmacy and the hospice agency results in a win-win outcome. The pharmacy wins by contracting directly with the hospice agency, providing improved clinical services for better reasonable reimbursement levels. The hospice agency wins with improved patient care and increased savings. Our program allows the pharmacy to dispense medications, actively participate in clinical programs and replace the third party administrator (TPA). Without interference from the TPA, savings are shared between pharmacy and hospice agency for improved patient outcomes.

Peter Kounelis
Peter Kounelis, Pharmacist

Peter Kounelis serves as Vice President, Elevate Provider Network. Mr. Kounelis has extensive experience in the pharmaceutical industry - particularly on the PBM side of the business. He has held a number of positions with increasing responsibility with Walgreens Health Initiatives (now Catamaran), MedImpact and AdvancePCS (now Caremark). He has extensive experience in launching clinical programs, consultative analytics services, financial modeling and contracting. Peter has additional experience in developing and commercializing medication adherence programs and pharmacy informatics solutions, for both managed care and pharmaceutical manufacturers. Peter earned his pharmacy degree at the Purdue University School of Pharmacy and his MBA at the Purdue University Krannert Graduate Business School of Management.

Mark has been with AmerisourceBergen since 1980. He has held several key roles within the company, including operations, sales and sales management. Most recently, Mark has been a critical member of the PBA team, where his success is driven by his knowledge of the business, both internally and externally. He has worked closely with our community pharmacy members to educate them on how to increase profits to their bottom line while lowering the medication cost to the hospice agency. Along with his team Mark has developed several programs for the pharmacy to work directly with the payers for increased re-imbursements and lower drug costs. 

Mark is a graduate of San Bernardino Valley College and Cal State San Bernardino, where he studied Communications and Business Management.

Kevin Komoto
Kevin Komoto, PharmD, MBA

Kevin is a well-respected pharmacist for a family-run, independent pharmacy network which has served the San Joaquin Valley of California since 1981. He received a dual doctorate of pharmacy and a master’s degree in business administration from the University of the Pacific. Following his pharmacy education, he completed a specialty residency with Kaiser Permanente in Contracting and Supply Chain Management. With a background in pharmacy and business administration, he has focused on expanding the roll of pharmacy, particularly in the area of community pharmacy.  

Kevin has been a support team member of the AmerisourceBergen Pharmacy to Hospice Administration (PHA) program since 2009, where he works with member pharmacies to:

-     Effectively utilize tools of the PHA program - "Peer to Peer"
-     Diversify service offerings to hospice organizations
-     Brainstorm ways to improve relationships with hospice organizations

Tana Cantor

Tana was hired by AmerisourceBergen in 2009 as a Pharmacy Audit Specialist. She developed and created ways for pharmacies to recoup money from the ever growing PBM audits with pre and post editing.

Most recently, Tana joined the PBA Services team and began working with the Pharmacy to Hospice Administrator program (PHA). Her main focus is to work closely with community pharmacy and educate them on how to increase their revenue while decreasing the drug cost to their local hospice agencies. Tana is the primary contact for on-boarding new PHA members, maintaining existing account relationships through account management and reporting.

Tana has 20 years of experience in the pharmacy related field working as a Pharmacy Audit Specialist, including background in Long Term Care/Skilled Nursing Facilities, Psychiatric Care Consultant, Home Infusion, Medical Records and PBM's.
